Complete our new patient forms online to streamline your child’s first visit and help us gather essential information for personalized care. For returning patients, please use our existing patient forms to update any necessary information, ensuring we have the most accurate details for your child’s ongoing care.
When you arrive, our front desk team will assist with check-in and any necessary paperwork, including new patient registration and medical history forms. We’re happy to help with the process to ensure everything goes smoothly.
Your child will meet our staff, followed by a quick tour of the equipment and patient area to ease nerves and spark curiosity. We’ll then take X-rays to get a detailed view of their teeth, checking for cavities and assessing how their adult teeth are developing.
After X-rays, your child will receive a professional cleaning to remove plaque and tartar. One of our doctors will then examine their teeth and gums for any concerns, including decay or orthodontic needs, ensuring their smile stays on track.

Please understand that your dental insurance benefit program is a contract between you, your employer, and your insurance carrier. We are not a party to that contract. You (not your insurance carrier) are ultimately responsible for payment.
Remember that “Covered” dental treatment is negotiated with the employer, so some necessary dental treatment may not be covered by your insurance carrier. Insurance companies often make up their own Maximum fee that they pay a percentage of. Usually, they will not share this information with us, so we have to make estimates that may be incorrect.
If you would like us to bill your insurance, we need accurate personal and insurance information. You may be asked to provide your social security number and copies of your driver’s license and insurance card.
*Accounts unpaid for over 60 days may accrue interest at 18% per year, or 1.5% per month. If your check is returned to us for Non Sufficient Funds, you will be charged a fee of $25.
